ATTENTION: Parents of 6th Grade Students

All 6th grade students need to have a Tdap (Tetanus, Diphtheria and Pertussis) and MCV4 (Meningococcal) vaccine before entering the 7th grade in the fall. This is a requirement of the Ohio Department of Health. The immunizations can be obtained at your physician’s office or at the Williams County Health Department. Please call ahead to make an appointment. The Health Department accepts medicaid and private health insurances, and no one is denied service for inability to pay.

After receiving the Tdap and MCV4 vaccine please send a copy of the updated immunization record to the school office. For questions or concerns call 419-633-6233.


ATTENTION: Parents of 11th Grade Students

All 11th grade students need to receive the meningococcal conjugate vaccine (MCV4) before entering the 12th grade. This is a requirement of the Ohio Department of Health. If your student received the first dose of the meningitis vaccine before they were 16, a second dose is required. If your student received a dose of MCV4 on or after their 16th birthday, they meet the requirement for school and do not need another dose. The immunization can be obtained at your physician’s office or the Williams County Health Department. Please call ahead to make an appointment. Medicaid and private health insurances are accepted at the Health Department, and no one is denied service for inability to pay.

The State of Ohio requires all students have these vaccines before entering the 7th and 12th grades. Therefore, your child will not be allowed to attend school without a record of it.

Medications at School

 

  • If your student needs to take a prescription medication while at school, your doctor will need to fill out a medication form.  These are available online or at the school office.  

  • If your student needs to take an over the counter medication at school, a parent will need to fill out a medication form and submit it to the school office.  These are available online or at the school office.

  • Medications must come to school in the original bottle from the pharmacy.  Medications sent in a baggie, envelope, or plastic container will not be accepted and the school will not be able to give these to the student.  

  • All medications will be given as instructed on the original bottle or as prescribed.

  • No medication of any kind is allowed on the school buses with the exception of emergency medications; Insulin, Inhalers, Diastat, and EpiPens.

  • Emergency medications, as noted above, can be carried by the student, unless it is felt that the student is not mature enough or responsible enough to take the medication without supervision.

  • All medications will be stored in the school office. The student will report to the office to take the medication.  Asthma inhalers, Epi-Pens, Diastat, and Diabetic supplies are the only medications that may be kept with the student or in their classroom.
